Welcome to our practice! At your first visit, Dr. Gum will conduct a comprehensive thorough assessment. A brief consultation explaining your dental diagnoses and treatment options will follow. We then will conduct a thorough dental cleaning on the same day.
You can help us by bringing the following information to your first visit:
- Dates of your last x-rays or actual X-rays (if you have them)
- Completed new guest forms (found on this site)
- A list of your current medical prescriptions
- Your insurance care if you have medical or dental insurance
- Your medical history. Please let us know if you have a medical condition such as diabetes, heart condition, high blood pressure, etc.
